Founded by Emerald Thomas, BSN, RN, U.S. Air Force Veteran, Summer Cares provides flexible PRN and per-diem services designed to enhance patient care and support agency operations.

Home Health Services include:

• Start of care and recertification visits

• Routine and on-call nursing visits

• Symptom management and chronic disease monitoring

• Caregiver education and medication reconciliation

• Wound care and patient assessments

Hospice Services include:

• Admissions and recertifications

• Routine and on-call visits

• Death pronouncements and end-of-life support

• Telehealth support for patient follow-ups and caregiver check-ins

Professional Credentials
All required credentials are current and available upon request, including:

• Active Texas RN license

• Professional liability insurance

• CPR/BLS certification

• W-9 and EIN for Summer Cares, PLLC

• Current immunizations and health records

• Proof of hospice and home health experience
